Herbst appliance – an effective tool in the treatment of severe Class II malocclusions. Biomechanics of treatment

Abstract
Introduction. Class II malocclusions are the most common occlusal disorders in children and young adults in Central Europe. The Herbst appliance is a fixed functional appliance designed for the correction of distoclusions. Aim of the study. To evaluate the efficiency of treatment with the Herbst appliance of Class II malocclusion based on the research results found in literature. The advantages of using fixed functional appliances as compared with removable ones have been presented. The construction, the operating principle and the effects exerted by the Herbst appliance on sagittal and dental changes as well as the TMJ are described. The most common indications for its use are listed in the paper and it was proved that the time when the therapy is initiated affects the outcome and stability of treatment. Material and methods. The review was carried out based on an indexed bibliography from medical data bases and the hand search at medical libraries. Twenty-five publications were selected from the literature, and they were used as the source for this study. Results. Based on literature it can be stated that from the clinical viewpoint only with the Herbst appliance therapy are we able to change the mandible growth processes to a large extent in a much shorter time (seven months on average) than the functional appliance therapy does. Treatment with the Herbst appliance is also effective at the end of growth period. Conclusions. The Herbst appliance therapy causes orthodontic and orthopedic changes in the face morphology and alveolar bone leading to an improved facial profile as well as the new bone formation and the remodelling of temporomandibular joints.
