İmmünokompetent Konukçu Bir Hastada Nokardiazis

Abstract
Aerobik bir aktinomiçez olan Nokardiazis hastalığından ender olan gram-pozitif bir bakteri olarak Nocardia cinsi sorumludur. Hastalıktan en sık akciğerler, santral sinir sistemi (SSS), ve deri etkilenir. SSS tutuluşu hastaların sıklıkla %20'sini oluşturur. Beyinde hemen her yer tutulabilir. Doku tanısı alınmadan önce yanlışlıkla primer ya da metastatik beyin tümörü tanısı alabilir. Nokardiazis tanısını, akciğer ve beyin tutuluşlarının bilinmesine karşın, koymak çok uğraştırıcı olabilir. Burada beyinden biyopsi ile kesin tanı almış hem beyin hem de akciğerde Nokardia abseleri ve çeşitli nörolojik semptomları olan bir erkek hasta sunmaktayız. Bu hasta intavenöz yolla, bir ay boyunca trimethoprim-sulfamethoxazole ile tedavi edildi ve idame tedavisi yine aynı ilaçla ağızdan 11 ay süreyle gerçekleştirildi. Hastanede kaldığı süreçte gelişen hidrosefalisi için hastaya bir şant operasyonu da gerekti. Bu olgu güncel literatür ışığında gözden geçirildi.
