Insulinemic Status and Nonalcoholic Fatty Liver Disease in Type 2 Diabetes Mellitus

Abstract

Introduction: In this article, non -alcoholic fatty liver disease (NAFLD) is discussed in detail. The epidemiology of NAFLD nonalcoholic steatohepatitis and the relationship of NAPLD with type 2 diabetes mellitus insulinemic resistance are broadly discussed. NAFLD incorporates histologically and clinically different non-alcoholic entities fatty liver (NAFL, steatosis hepatis) and steatohepatitis (NASH characterized by hepatocyte ballooning and lobular inflammation + fibrosis) might progress to cirrhosis and rarely to hepatocellular cancer. Type 2 diabetes mellitus (T2DM), insulin resistance (IR), obesity and NAFLD are particularly closely related. Type 2 diabetes mellitus (T2DM) and nonalcoholic fatty liver disease (NAFLD) commonly exist together. Age is major determinant of NAFLD. As Body mass index and waist-to-hip ratio were not independently associated with nonalcoholic fatty liver diseases in our study, suggesting that obesity might also be effects of insulin resistance. Insulin resistance leads to foot accumulation in hepatocytes by lipolysis and hyperinsulinemia. Objective: The objective of the study was to find out the association of NAFLD with insulinemic status in type 2 DM subjects. Results: The present data leads to the suggestion that there is a strong association between insulin resistance and NAFLD. However, the question of whether hyperinsulinemia or insulin resistance is the primary disorder of NAFLD remains unangered. Further studies are needed to elucidate interrelationship between NAFLD, metabolic syndrome and insulin resistance.
