Interdisciplinary treatment of traumatically lost maxillary central incisor – a case report

Abstract

In children and adolescents who lost at least one upper incisor as a result of trauma the choice of treatment option depends on patient’s age, type of trauma, condition of surrounding tissues, occlusal relationships and growth pattern. The following options should be considered: orthodontic space closure; provisional removable prosthetic restoration, to be later replaced by a permanent solution, including an implant-based prosthesis or autogenous transplantation. Autogenous transplantation has become the standard technique, alongside single dental implant. The advantage of this method is associated with the use of the patient’s own natural teeth. Its drawback is the risk of ankylosis and rarely root resorption of transplanted tooth. Children aged 7 to 10 are at the highest risk of trauma, hence they comprise the largest group of patients who need such intervention. Transplanted tooth preserves the growth potential of the alveolar ridge (in contrast to implant) and supports the development of a new alveolus. Moreover, the formation of a new periodontal ligament making such teeth amenable to conventional orthodontic movement. Most authors recommended initiation of orthodontic treatment 3 to 6 months after transplantation. The aim of this paper was to present joint orthodontic and surgical treatment of a 8 year old girl with post-traumatic loss of a maxillary central incisor (Ellis class V trauma). (Orthod.
