Isolated ovarian tuberculosis mimicking ovarian carcinoma

Abstract
Although genitourinary tuberculosis is common, reports of isolated ovarian tuberculosis is rare. However, its presentation can mimick that of an ovarian tumour. A woman of 26 years presented with abdomen pain & distension of abdomen for 1 week with CECT findings of well defined peripherally enhancing soft tissue lesion with cystic component in the left adnexa with significantly elevated CA-125 level. A provisional diagnosis of left ovarian carcinoma was made and diagnostic laparoscopy & proceed was planned. On laparoscopy the left ovarian mass was seen with presence of ascitic fluid. Biopsy was taken & ascitic fluid was sent for cytological studies. Postoperative histopathological studies, however, revealed epitheloid histiocyctes and Langerhans giant cells compatible with tuberculosis, with no signs of malignancy. Ascitic fluid cytology showed no evidence of malignant cells. Antituberculosis treatment was started and patients symptoms resolved with decrease in CA-125 levels. Isolated ovarian tuberculosis is most common in young women living in endemic zones. CA-125 levels can be raised in both conditions and imaging is not conclusive. Early diagnosis of ovarian tuberculosis is vital as untreated disease can lead to infertility.
