Abstract
Na podstawie przeglądu klasycznego i aktualnego piśmiennictwa dotyczącego ewolucji w klasyfikacjach kręgosłupa piersiowo-lędźwiowego. Przedstawiono zalety i wady podziałów Denisa i AO. Szczegółowo omówiono najnowszą klasyfikację tych złamań zaproponowaną przez Vaccaro (TLICS - Thoracolumbar Injury Classification Severity Score). Zwrócono szczególną uwagę na zjawisko istniejącej niestabilności w tych złamaniach. Zwięźle omówiono zasady nowoczesnego postępowania operacyjnego. Stabilizacja krótkoodcinkowa przeznasadowa i usztywnienie jest postępowaniem ogólnie przyjętym w złamaniach niestabilnych i złamaniach z ubytkami neurologicznymi.
