Lipid metabolism in skin of patients with psoriasis and prospects for studying the significance of its disorders in the pathogenesis of dermatosis

Abstract

Objective — to analyze the characteristics of the spectrum of lipids in the skin of patients with psoriasis and determine the value of lipid metabolism disorders in the pathogenesis of dermatosis. Materials and methods. The features of lipid metabolism in patients with psoriasis were systematized and described based on the analysis of literature sources and our own preliminary research (clinical, biochemical, immunopathohistochemical). Results and discussion. Metabolic disorders are significant pathogenetic factor of the psoriatic process, and some of them act as triggers. Conclusions. Currently, the biochemistry, morphology, physiology and pathophysiology of human skin lipids has been studied in sufficient detail. These data make it possible to take a fresh look at the pathogenesis of psoriasis and approaches to its treatment. At the same time, the regulation of the organization of the lipid lamellar membranes of the stratum corneum to determine the primary or secondary nature of lipid disorders accompanying psoriatic disease need further study. It is also important to determine how cholesterol and phospholipids are transported to lamellar bodies (LB) and what specific carriers are needed for this, as well as to study how the transport of lipids and proteins to LB is regulated. It is necessary to identify the molecular processes necessary for the transport and secretion of LT, to clarify the effect of PAR2-receptors on the processes of secretion of LT and rooting, as well as indicators of the epidermal barrier on the activity of enzymes responsible for the final stages of cholesterol biosynthesis. To determine the effect of lipid metabolism in the skin on the pathogenesis of psoriasis, it is important to identify the enzymes involved in the synthesis of phospholipids and triglyceride lipolysis in the epidermis, to study how they are regulated, and also identify endogenous ligands for the receptors of PPAR, LXR and PXR classes in the epidermis of the skin affected with psoriatic rash.
