Localised Castleman's disease of benign clinical cours

Abstract

SUMMARY Introduction: Castleman's disease is defined as enlarged follicules with the presence of abnormal germinal centres, vascular proliferationand endothelial hyperplasia. There are three histopathological types: hyaline-vascular, plasma celt and a mixed type. When analysing the clinical course one can identify two forms: solitary and multicentric. Mostly the changes occur in the lymph nodes of mediastinum, lungs, neck and axilla, but the abnormal masses can be found in any lymph node within a human body. Etiopatogenesis of the disease is sill unknown. Total excision of the lesion is the best treatment of solitary form of the illness. The treatment of the multicentric form of the disease is stili not determined. Case presentation: We present the case of 28-year-old patient that was treated in ENT Department of Międzyleski Szpital Specjalistyczny in Warsaw in 2010 because of the cervical tumour with 13-year history. The definite diagnosis was established by histopathologic confirmation after surgical resection of the lesion. Discussion: The Castleman's disease is a rare cause of localized or general lymphadenopathy. There is no proved clinical research that clarifies the doubts related to the illness. The diagnostics of the disease is difficult and has to be performed with strict cooperation of physician, radiologist and patomorfolo-gist. The recovery ratio in patients with localized form of hyaline-vascular type reaches 100%. Because of rare incidence of multicentric form of plasmatic celt type the research comparing the efficiency of terapeutic trials is difficult to plan. Accordingly, the definition of the standards for long-term treatment has not been provided.
