Malrotation of gut with primary sclerosing cholangitis: an unusual presentation

Abstract
Malrotation of gut is a familiar paediatric condition presenting in the neonates. Adults however present with obscure clinical symptoms, such as recurrent abdominal pain and vomiting often resulting in multiple hospital visits and posing a diagnostic dilemma. We report the case of 22-year-old female experiencing infrequent episodes of abdominal pain and distension, recurrent vomiting and constipation that was diagnosed with malrotation of gut and primary sclerosing cholangitis. Contrast-enhanced CT revealed typical whirlpool sign or inverted relationship of superior mesenteric artery and vein. Timely recognition of this condition can save the patient from disastrous consequences, such small bowel gangrene.
