Management in cases of palatally impacted permanent canine.

Abstract
AIM: to describe differential diagnostics and orthodontic-surgical management in cases of palatally impacted canines.MATERIAL AND METHODS: Records of 20 patientswith 23 impacted permanent canines were collected.A carefull interview, clinical examination, and pantomogramanalysis were conducted. Depending on the severityof condition, two groups were established: superficial anddeep impaction– each with their own specificity. First groupcomprised the patients with following characteristics: presenceof palatal bulge confirmed by palpation, pantomographicimage of impacted canine cusp projecting to thecervical area of neighboring teeth. Second group was characterized by: absence of visible and palpable bulge on palate,and impacted canine cusp projection to the vicinity ofneighboring teeth apices.RESULTS: Among 23 palatally impacted permanentcanines 15 (65%) were qualified as superficial and 8 (35%)as deep impaction cases. Majority of patients were females(85%). The mean age was 17-3. Abnormality occurred significantlymore often on the left side of the palate. A persistentprimary canine was observed in 74%. In most ofcases they did not show any mobility. Reporting for theorthodontic treatment, 12 patients were aware of incorrectlocalization of the tooth. Remaining 8, unaware of problem,reported for occlusion correction and the impactedtooth was revealed by chance during specialistic examination.Orthodontic-surgical management was different inboth groups depending on depth of the canine impactionCONCLUSIONS: Management in palatally impactedpermanent canines depends on severity of impaction. Insuperficial impaction preceding surgical exposure is recommended.Treatment with fixed appliance is done aftercanine eruption in the palate. Deep impaction cases requirepreliminary treatment with fixed appliance, intraoperativebonding of orthodontic bracket and an immediateapplication of orthodontic force. (Orthod. Forum 2005;1:176-83)
