Metabolic Syndrome in Cerebrovascular Diseases

Abstract

Objective: The term "metabolic syndrome" has been proposed to define a cluster of several risk factors for cardiovascular disease. We investigated the existence of metabolic syndrome in acute period cerebrovascular patients and its relationship with acute mortality. Methods: Ischemic cerebrovascular patients were grouped on the basis of the "Trial of Org 10172 in Acute Stroke Treatment" study; and the ones with a specific cause, cryptogenic stroke, and inconclusive cases were not included in the study. Total number of patients was 474 of which 354 were ischemic (including 99 cardioembolic, 147 large arterial atherosclerotic, 108 small vessel occlusion) and 120 were hemorrhagic. We decided metabolic syndrome in the presence of three or more of five items of the diagnostic criteria of the "National Cholesterol Education Program Adult Treatment Panel III " . Results: The incidence of metabolic syndrome in ischemic patients (67,2%) was significantly higher than hemorrhagic ones (33,3%) (p<0,001). Metabolic syndrome in the cardioembolic group (49,5%) was found to be significantly lower than the non-cardioembolic ischemic group [large arterial atherosclerotic (74,8%), small vessel occlusion (73,1%)] (p<0,001). Pertaining to acute mortality, the metabolic syndrome did not show any correlation with cardioembolic, large arterial atherosclerotic and small vessel occlusion; whereas it was found to be significantly higher in hemorrhagic patients (p<0,001). Conclusions: This study, which has been running on, made us think that the incidence of metabolic syndrome was greater in ischemic cerebrovascular disease than intracerebral hemorrhage, and it might have a role for acute mortality in intracerebral hemorrhage.
