Multiple impacted teeth in the anterior section – an orthodontic and surgical problem. Case report

Journal Title: Forum Ortodontyczne - Year 2017, Vol 13, Issue 3

Abstract

Such dental abnormalities like transposition and impacted teeth in the anterior maxilla are rare but serious disturbances. They are diagnosed early due to their localisation. Reasons responsible for such abnormalities include systemic factors (endocrine disturbances, systemic diseases, ionising radiation, genetic factors); local factors (supernumerary tooth, odontoma, no space in the dental arch, persistent deciduous tooth) as well as traumas at the early age that may cause damage to a permanent tooth germ and its dislocation within the alveolar process. Aim. Presentation of multidisciplinary orthodontic and surgical treatment of a female patient with transposition of the lateral incisor and canine, and impacted medial incisor in the maxilla. Material and methods. It is a case of orthodontic and surgical treatment of a 9-year-old female patient, including surgical exposure and gradual introduction of the impacted tooth 11 and correction of transposition of the right lateral incisor and canine using an early 2x4 appliance followed by a full orthodontic appliance. Results. As a result of treatment mechanics applied it was possible to restore an order of teeth positions in a dental arch, canine guidance on the side of a defect, achieve functional occlusion and smile aesthetics. Conclusions. Early diagnostics and treatment of this abnormality in teeth development was vital and had a significant effect on the prognosis. As treatment of this anomaly is complex and long a well-developed treatment plan that considers a possibility of complications is a must, as well as good cooperation between a doctor and a patient with their parents. (Kiziewicz P, Grochowska-tatarczuk M, Grodzka I, Szarmach J, Szarmach IJ.
