A COMPARATIVE STUDY OF DCR VERSUS TOTI'S PROCEDURE

Abstract
Dacryocystitis is a constant menace to the eye because the dacryocystitic fluid contains various pathogenic bacteria such as pneumococci, staphylococci, streptococci etc. Therefore, hypopyon corneal ulcer can be easily developed even with minute corneal abrasions. The danger of panophthalmitis is also present if any intraocular surgery is undertaken hence, an effective treatment of dacryocystitis is very necessary. This prospective study was conducted in the Department of Ophthalmology, G.R. Medical College, Gwalior from June 2008 to June 2009. Sixty patients that constituted study material were divided into two groups. Group A: 30 cases that underwent Conventional DCR. Group B: 30 cases that underwent TOTI’s Procedure. The predominant age group was 30 – 50 years i.e. in 4th and 5th decade. Females were affected more than the males with ratio of 4:1. Lower socioeconomic class was most frequently involved in both groups. Syringing three month postoperatively showed patent passage in 28 cases (93.34%) and 26 cases (86.66%) in group B. Toti’s procedure, although less successful than DCR, is technically easier, less time consuming, and is particularly useful in certain circumstances where DCR cannot be done.
