A STUDY ON STRESSFUL LIFE EVENTS AND SOCIO-DEMOGRAPHIC PROFILE OF MENTALLY-ILL PATIENTS ATTENDING INSTITUTE OF MENTAL HEALTH (IMH), HYDERABAD, ANDHRA PRADESH.

Abstract
[b]BACKGROUND[/b]: The WHO global burden of Disease study estimates that mental and addictive disorders are among the most burdensome in the world, and their burden will increase over the next decades. The mental and behavioural disorders account for about 12% of the global burden of disease. Especially emotional (or) behavioural symptoms may occur in response to stressful life events in their routine life. According to DSM IV (Diagnostic statistical manual IV) if the symptoms (or) behaviours appear within 3months of the onset of the life change (or) stressful life events and are clinically significant. However, these estimates and projection are based largely on review of literature. In India it is evident that till now the study with regard to mental health especially in relation to stress is for a few. Keeping this in mind study has been planned to look into specific factors involved in the mental health Eg: socio-demographic factors involved in stressful life events, study of causal relationship between stress and mental health. [b]OBJECTIVES[/b]: To study the socio-demographic factors and presumptive stressful life events among mentally ill persons and to assess the causal relationship of the presumptive stressful life events of mentally ill persons. [b]METHODOLOGY[/b]: The study subjects were drawn from the outpatient department, of Institute of Mental Health (IMH) Hyderabad, attending for the 1st visit with less than 1 year duration of symptoms and were randomly selected for the interview with semi structured questionnaire and presumptive stress full life event scale. The interview was conducted after the patients were examined and diagnosed by the psychiatrist. Data collection for a period of 9 months, compilation and analysis for 3 months. The PSLE scale designed by Gurmeet singh et al to suit Indian population along the lines of Holmes and Rahe’s social readjustment rating schedule. [b]RESULTS[/b]: Among the total mental illness study population 56%were males,44%were females, majority i.e.,( 38.66%) of them were 23-42years age group and less illness with > 45years of age i.e., (4.62%), mean age is 29.63yrs, belong to rural(54.67%), nuclear family(62%).Married group(58.66%), it is showing the statistical significance between groups in marital status, Chi Sq.=6.204.P=0.045.(<0.05). Living with family(85.33%), most of them were from low socioeconomic (68.01%) than upper class (5.33%) highly significant, both illiterates and literates were equally distributed, majority of the males having multiple addictions (45.25%) i.e smoking, alcohol, pan chewing etc. PSLE scale represents the family conflicts (53) 35.33% are the first major stressful life events, in their mental illness, financial problems (45) 30%, unfulfilled commitments (35) 23.33%, conflicts with in laws (31) 20.66%, followed by Change in sleeping habits (30) 20% and majority (66.67%) were presented with psychotic symptoms in between psychotic and neurotics statistically significant. (Chi-sq: 5.966: p=0.015). Majority of the patients duration of the illness was 1m-6months, (53.33%), only (8.66%) were <1week. Majority of the patients were provisionally diagnosed as Depression (28%), followed by syndromic approach (26%).
