Abstract
Objective:IdentifytheincidenceofPetersen'sherniaingastricbypasspatientsatourinstitution.Summary:Laparoscopicgastricbypasshasprovedtobethebesttreatmentforobesityandtheresolutionoftheircomplications,however,itpresentslong-termcomplicationssuchasPetersen'shernia.Methods:Aretrospectivestudyofasinglecenter,withatotalof225gastricbypassperformedinthebariatricandmetabolicsurgeryclinicoftheHospitalCivildeGuadalajara“Dr.JuanI.Menchaca”fromJanuary2014toMarch2020.Results:Ofthe225operatedpatients,aglobalfollow-upof80.44%wasobtained,withanaveragepostoperativefollow-upof31(+/-20.92)months,itwasfoundthatin10patients(4.44%)presentedasymptomaticinternalhernia.Ofthese,9patientswasadmittedtotheemergencyroomand1detectedintheoutpatientclinic.ThesepatientspresentedatthetimeofsurgeryaBMIof27.65(+/-5.01)withapercentageofexcessweightlostof62.35%(+/-25.60).Thelaparoscopicapproachcouldbecompletedin100%ofthecases.Regardinghospitalstay,anaverageof1.1(+/-0.31)dayswaspresented.Notrans-surgicalcomplicationsormortalitywerereportedinthegroupstudied.Themeantimebetweenprimarysurgeryandreoperationwas21.3(+/-12.98)months.Conclusion:Ina6-yearperiodwithaglobalfollow-upof80.44%,wherethePetersendefectwasnotclosed.Anincidenceof4.44%wasfound,withanaveragefollow-upof31(+/-20.92)months.However,thepercentageofPetersen'sherniamaybeunderestimatedsinceonlythosewithsymptomsthatleadthepatienttoemergencycareareusuallydiagnosed,soahighindexofsuspicionmustalwaysbehad.
