NON-SPECIFIC (PYOGENIC) SPINAL INFECTIONS

Abstract

Omurga enfeksiyonlar›, vertebra cisimlerini, intervertebral diskleri, spinal kanal› ve paravertebral yumuflak dokular› etkiler. Çok s›k görülmemekle beraber tan› koymadaki zorluklar tan›n›n gecikmesine ve çok ciddi sonuçlar›n ortaya ç›kmas›na yol açmaktad›r. Gecikmifl tan› omurga deformitesine, önemli nörolojik komplikasyonlara hatta ölüme sebep olabilir. Yo¤un çal›flmalara ra¤men omurga enfeksiyonlar›n›n s›n›fland›r›lmas›, tan›s› ve tedavisinde hala kar›fl›kl›klar söz konusudur . Osteomiyelitin ortaya ç›k›fl› hastan›n yafl›, immün cevab› ve etken mikroorganizmaya ba¤l›d›r. Uygun flartlar alt›nda hemen hemen tüm enfektif organizmalar vertebral enfeksiyona yol açabilirler. Piyojenik formda en s›k etken Staphylococcus aureus’ tur. Bunun d›fl›nda koagulaz negatif stafilokoklar, Enterobacteriaceae ve di¤er Gram-negatif basiller, aerob Gram-negatif basiller (Pseudomonas aeruginosa gibi), hemolitik streptokoklar, Haemophilus influenzae, Streptococcus pyogenes, Salmonella türleri taraf›ndan meydana gelir. Omurga enfeksiyonlar›, ciddi kas-iskelet ve nörolojik komplikasyonlar›n engellenebilmesi için erken evrede tan› konmas› gereken a¤›r bir patolojik durumdur. Klinik bulgular aç›s›ndan enfeksiyondan flüphelenildi¤i anda antimikrobiyal tedavinin en iyi flekilde yap›labilmesi için mikrobiyolojik kültür ve duyarl›l›k testleri uygulanmal›d›r. Mikrobiyoloji teflhiste alt›n standartt›r ve tan›y› do¤rulamak için mutlaka gereklidir.
