Optimization of Ultrasonography in Assessing the Heart and Blood Vessels Condition in Patients with Arterial Hypertension with or without Type 2 Diabetes Mellitus

Abstract

The objective — to study the peculiarities of the heart and blood vessels condition in patients with essential hypertension (EH) with or without type 2 diabetes mellitus. Materials and methods. The study involved 134 patients: the first group — 44 patients with II stage EH, grade 2 and 3, the second group — 45 patients with II stage EH, grade 2 and 3, associated with type 2 diabetes mellitus, control group — 45 healthy volunteers. The groups were comparable in age, sex, risk factors for cardiovascular complications. The methods of ultrasonography and multivariate statistical methods were used. Results. Using the factor analysis as a method of multivariate statistics, among 140 ultrasonographic indicators, which are recommended by the European Society of Echocardiography, 14 indicators were selected, and four integrated indicators were created on their base. Their clinical interpretation shows that these indicators characterize the functional state of the heart, macrocirculation, microcirculation and structural condition of the heart. At the same time, the indicators of structure and microcirculation may be considered as type 2 diabetes mellitus markers. Conclusions. With the help of factor analysis as a method of multivariate statistics, new combinations of ultrasonographic indicators were obtained in terms of factors, which are the systemic description of the target organs response on comorbid pathology: remodeling of the main blood vessels and microvessels, structural and functional cardiac remodeling. Holistic, multidimensional concept of the remodeling processes is possible only taking into account values of all indicators that create the factor.
