Orthodontic extrusion in the interdisciplinary treatment based on literature review and personal experience

Abstract
Introduction: Growing awareness of the benefits arising from the preservation of natural tooth tissues and related structures challenges clinicians to undertake treatment of difficult cases, that often combine knowledge from different dental disciplines. Orthodontic extrusion is an example of such a treatment, which utilizes vertical tooth movement and subsequent periodontal. The aim of the study is to present, based on literature review and personal experience, applicability of this treatment method in the interdisciplinary treatment, with particular emphasis on periodontal, implant and prosthetic treatment. Material and methods: Application of extrusion was described based on selected cases. The treatment plan included extraction of the tooth followed by the implant treatment. After extrusion of the tooth to the limits of the orthodontic extraction an implant was positioned in the place of the tooth which was reconstructed with prosthetic crown. In the second case clinical examination revealed dental hard tissue border extending under the gingiva. After the extrusion the prosthetic reconstruction with the post and prosthetic crown was introduced. Discussion and summary: Personal experience confirm literature data that orthodontic extrusion is considered as a conservative treatment, which helps to maintain the tooth and avoid consequences of extraction as bone loss and necessity of teeth preparation for fixed prosthesis. Introducing the extrusion procedure also enables to avoid surgical clinical crown lengthening, which is being employed for similar indications and is associated with the loss of alveolar bone and the additional factors limitating this method concern aesthetics. It can be stated that orthodontic extrusion is an interdisciplinary issue which allows to resolve complex clinical situations and can be found as application in various fields of dental sciences.
