Orthodontic-surgical treatment of partially impacted second molars in the maxilla – a case study

Abstract

Impacted second molars are observed relatively rarelyand their incidence is from 0.03% to 2.3%. Thisphenomenon is more frequently observed in themandible than in the maxilla. Aim. The work aimed toassess results of orthodontic and surgical treatment ina patient with partially impacted teeth 17 and 27.Material and methods. An 18-year-old male patientwith a treatment plan including surgical extraction ofteeth 18 and 28, partial luxation of teeth 17 and 27 andtheir uprighting and bringing down teeth 17 and 27with sectional archwires and skeletal anchorage.Results. Active orthodontic treatment was conductedfor 3.5 months, and completely satisfactory outcomeswere obtained, namely uprighting and improved positionof the second molars in the maxillary arch. This strategywas followed by retention treatment for 6 months. Oneyear after the end of orthodontic treatment therapeuticoutcomes were assessed as stable. Conclusions.Surgical-orthodontic treatment without extraction ofthe second molar seems to be the best managementmethod to allow for a correct occlusal position of a toothand preventing from periodontal defects.
