Osteoblast activation factors in HIV-infected patients on antiretroviral therapy

Abstract
<b> Objectives.</b>Bone diseases: osteoporosis, osteopenia and aseptic bone necrosis are relatively common in HIV-infected patients on combined antiretroviral therapy (cART). Pathogenesis of this abnormalities on cART are still unclear. We measured changes of biochemical factors of osteoblast activation: parathyroid hormone (PTH) and osteocalcin (BGP) serum concentrations in patients on ART.<br><b> Material and methods.</b> In 36 patients(pts) without clinical symptoms of bone diseases, before and after one or two years of ARV, retrospectively PTH and BGP serum concentrations, using Diagnostic Systems Laboratories ELISA tests, were measured.<br><b> Results. </b> Of 36 patients involved in the analysis (mean age: 40.4 ± 10.5 yrs), 11 (31%) were female and 25 (69%) were male. Elevated serum PTH level in 4 (11%) pts before and 7 (19%) pts past one or two years of ART were detected. Mean serum PTH concentration before ART. (PTH 1): 22.9 ± 28.5 pg/ml and after start of ART (PTH 2): 33.5 ± 34.0 pg/ml were assessed. Osteocalcin mean serum level before ART (BPG 1): 8.4 ± 7.9 ng/ml and past start of ART (BGP 2): 15.4 ± 12.7 ng/ml were measured. Increase of parathyroid hormone and osteocalcin concentrations in treated pts were statistically significant in Wilcoxon matched pairs tests (p<0.05).<br><b> Conclusions.</b> Examination of PTH and osteocalcin serum concentrations during currently used antiretroviral therapy may be helpful in early detection of bone mineralization abnormalities in HIV-infected patients without clinical symptoms of bone diseases. Standard method used for diagnose osteopenia and osteoporosis is the bone mineral density (BMD), measured with dual X-ray absorptiometry (DEXA).
