Patellofemoral Osteoarthritis - Conservative Non-Pharmacological Treatment A Prospective Study

Abstract

The main purpose of this study was to determine possible pain mechanisms in Patellofemoral osteoarthritic patients and to assess the results of conservative non pharmacological management. A total of 251 patients were enrolled. Out of which 120 patients had patello femoral osteoarthritis with excessive lateral pressure syndrome and 131 had patello femoral osteoarthritis without excessive lateral pressure syndrome. The age group was between 15 to 75 years. The average follow up was done up to 18 months. Up to the age of thirty years most of the patients were of chondromalacia with minor patellofemoral osteoarthritis. At 6 month of follow up complete subjective, functional, and clinical recovery had occurred in almost 100% of the patients in this age group. Over the age of 40 years; at six months of follow up complete subjective, functional, and clinical recovery had occurred in almost 50% of the patients, rest 50% had minor recovery even at 18 months, but the disease severity became lowered significantly. Majority of patients with isolated patello femoral degenerative changes do not need surgical treatment. Those who are symptomatic can usually be successfully treated with weight loss and quadriceps strengthening exercises, patella taping, patella bracing and physiotherapy. The one and half year overall outcome was good in approximately two-thirds of the patients. However, the remaining patients still had symptoms or objective signs of a patello femoral abnormality but their severity became lowered significantly.
