Peculiarities of the carbohydrate metabolism in patients with chronic obstructive pulmonary disease and concomitant chronic pancreatitis

Abstract
Background. Chronic obstructive pulmonary disease is a leading cause of morbidity and mortality in the world. We can see a combination of chronic obstructive pulmonary disease with disorders of the digestive system, such as chronic pancreatitis, in clinical practice. The functional status of the pancreas plays a direct role in the development of the main components of the metabolic syndrome (hyperinsulinemia, insulin resistance with impaired glucose tolerance). Materials and methods. We have examined 90 patients with chronic obstructive pulmonary disease and chronic pancreatitis, and studied of carbohydrate metabolism, lipid peroxidation and the content of final metabolites of nitric oxide (nitrites/nitrates). Results. The study found an increase in the degree of insulin resistance and the development of oxidative stress. Comprehensive treatment of patients with chronic obstructive pulmonary disease and chronic pancreatitis, which included the administration of basic therapy and reduced glutathione, led to a decrease of the insulin resistance and the restoration of serum reduced glutathione. Conclusions. Decrease in the serum reduced glutathione and the increase of serum level of final metabolites of nitric oxide is accompanied by the increase of degree of insulin resistance.
