Perforator based flaps in leg reconstruction

Abstract
Background: Perforator flaps have recently become ubiquitous in the field of plastic surgery. To understand and appreciate their unique nature, it is necessary to compare and contrast them with the development of other types of flaps. Aim: To study the various possible perforator based flaps in reconstruction of defects at various levels of leg Methods: This prospective study was done in 20 patients with post traumatic open fractures with exposed bones and implants or external fixator in the leg. A hand-held ultrasound Doppler was used preoperatively and intraoperatively to detect the perforator vessels. Results: Right leg was involved in 12 cases and left leg in 8 cases. The commonest cause of the defect in our study was trauma particularly road traffic accidents. The smallest defect was 3cm x 1.5 cm, and the largest defect was 19 x 12 cm which needed flap cover. Major complications like flap loss occurred in 10% of our cases which were treated with the second flap. Conclusion: For small and moderate-sized defects particularly in the lower third leg, perforator based fasciocutaneous flaps are the good option in reconstruction which may even replace free flaps.
