Porażenie nerwu międzykostnego przedniego po nadkłykciowym złamaniu kości ramiennej u osoby dorosłej. Studium przypadku

Abstract
Przedstawiamy przypadek porażenia nerwu międzykostnego przedniego po nadkłykciowym złamaniu kości ramiennej u 24-letniej kobiety. Pacjentka diagnozowana i leczona kolejno w dwóch szpitalach, gdzie podczas standardowego badania nie zwrócono uwagi na upośledzenie funkcji ręki wskazujące na uszkodzenie nerwu. Wykonano zabieg otwartego nastawienia ze stabilizacją złamania z użyciem dedykowanych płytek LCP i śrub, uzyskując ostatecznie dobry wynik leczenia. Porażenie opisywanego nerwu wykryto po 14 dniach przy okazji pierwszej wizyty kontrolnej. Zdecydowano o leczeniu zachowawczym. Po ok. 8 miesiącach uzyskano pełen powrót funkcji ręki. Uważamy, że tego typu uszkodzenia zdarzają się częściej, lecz z powodu niedokładnego badania fizykalnego mogą zostać łatwo przeoczone. Ze względu na dobro pacjenta oraz aspekty prawne mogące wynikać z braku właściwej diagnozy, należy rozważyć wprowadzenie udoskonalonych algorytmów badania klinicznego chorych po tego typu złamaniach.
