Prevalence of depression and risk factors among women in Poonamalle,Tamilnadu,India

Abstract
Aims: Depression is an important public health problem causing considerable morbidity and disability worldwide. Its burden among women especially in their productive age group in rural area is less known. This study aimed to determine the prevalence of depression and evaluate its associated risk factors in a rural setting Methods and Material: We adopted a healthcare facility based cross-sectional study. We recruited 378 participants aged between 18 to 45 years, who had visited the Poonamalle Health Unit , Tamil Nadu, for any of their health concern during January 2014 to April 2014 . We assessed their socio-demographic profile using a structured questionnaire and psychiatric morbidity, using the Becks depression inventory (BDI) structured assessment tools.. Results: Among the 378 participants the mean age was 35 [± 10] years. The prevalence of depression was 20% ,[ 95%CI :15.8 – 24.0 %]. Among the participants 90% of them were married , 79.1% were homemakers and 90% had a monthly family income of Rs.4000. Two thirds (60.6%) of the respondents live in their own houses. The risk factors for depression were low income (AOR 2.1; 95% CI 1.3–4.2.), living away from healthcare facility (AOR 1.1; 95% CI 1.5–2.5), nuclear family type (AOR 1.3; 95% CI 1.1–2.2), family illness (AOR 1.2; 95% CI 1.1-2.1) pre-menstrual syndrome (AOR 1.9; 95% CI 1.5–2.6),History of physical illness (AOR 1.1; 95% CI 1.4-2.7),substance abuse in family (AOR 1.6;95% CI 1.1-2.9). Conclusions: There was a high prevalence of depression among rural women . Poverty, ,nuclear family type, physical ill health, premenstrual syndrome, substance abuse in family members were the main risk factors for depression. This study highlights the need for developing mental health screening programme for early identification and targeted psychosocial interventions to minimise the burden and enable the government to adopt policies to integrate mental health with the primary care system.
