Abstract

The first essence of adolescence in a female is the onset of menarche. But as the plight of women is it is associated with number of problems and the most common among them is dysmenorrhoea. Primary dysmenorrhoea is by far the most common gynaecological problem in menstruating women. Being a very common disorder that we come across in our day to day practice but still it is under-diagnosed and under-treated. Dysmenorrhea is defined as cyclical lower abdominal / pelvic pain during menstruation which radiates to the back or the thigh. Dysmenorrhea is derived from greek words “dys” meaning difficult, “meno” meaning flow. It is of two types primary and secondary. Primary being the most common and ill understood is defined as cramping pain in the lower abdomen that occurs at the onset of menstruation in the absence of any identifiable disease. The most well accepted theory is that of increased level of prostaglandin (PG) F2α that causes intense uterine contractions, reduced uterine blood flow, and increased peripheral nerve hypersensitivity resulting in severe pain. There is a wide spectrum of clinical presentation comprising of colicky supra pubic pain, lumbosacral backache radiating to anterior thigh, nausea, vomiting, diarrhoea and rarely syncopal attack etc. Being a very common disorder that we come across in our day to day practice but still it is under-diagnosed and under-treated. Thus we need to keep this gynaecological problem in mind while dealing with young patients so that it gets diagnosed and treated in every case that we come across.
