Primary Percutaneous Coronary Intervention (PPCI) in Acute Myocardial Infarction Complicated by Cardiogenic Shock in a Newly Emerging Cardiac Centre in Nepal
Journal Title: Nepalese Heart Journal - Year 2009, Vol 6, Issue 1
Abstract
Background: Acute myocardial infarction (AMI) is complicated by cardiogenic shock in 7-10% of patients. Mortality rate is exceedingly high and reaches 70-80% in those treated conservatively. Large thrombolytic trials demonstrate 60% mortality with most effective thrombolytic agent. In between September 2005 to August 2008 total PCI in Shahid gangalal National Heart Centre (SGNHC), Nepal was 452. among them primary PCI (PPCI) in AMI with cardilgenic shock was done in only 16 patients with cardiogenic shock who underwent PPCI, 6 patients are in out of 50% (n=8) patients with cardiogenic shock who underwent PPCI, 6 patients are in routine follow up over 12 months and 2 were doing well in subsequent 6 months but lost in follow up afterwards. Primary PCI in AMI complicated by cardiogenic shock has lower mortality and improved outcome. High cost, high in-hospital mortality, need for trained manpower are the major limitations.

Rheumatic Heart Disease in Nepal: Current Scenario
Rheumatic Heart Disease, though declining in developed world, is still prevalent in Nepal in spite of increased awareness and improvement in preventive programs. This article presents the current scenario of Rheumatic...
Aortic Arch Aneurysm Repair under Deep Hypothermic Circulatory Arrest
A 46 years female underwent aortic arch repair surgery under deep hypothermic circulatory arrest (DHCA) and right antegrade cerebral perfusion. The patient was extubnated after 13 hours of the surgery and discharged fro...
Multivessel primary PCI in patient with peripheral arterial disease
Thrombosis of multiple coronary arteries is extremely rare fnding in the setting of acute myocardial infarction. This often leads to cardiogenic shock and sudden cardiac death. We report a case of 61 years old gentleman...
Coronary Artery Thrombosis: Is High Serum Homocysteine Level a Culprit?
Hyperhomocysteinemia is a risk factor of coronary artery disease associated to arterial thrombosis. We report a case in which coronary angiography showed full of thrombus causing total obstruction of the left anterior...
Predictors of in-hospital mortality following mitral or double valve replacement for rheumatic heart disease
Backgrounds and Aims: Factors affecting outcome of mitral valve replacement in rheumatic population of Nepal is unknown. The aim of this study was to identify the predictors of in-hospital mortality in patients undergo...
