Prognostic value of body weight loss and tissue components in patients with chronic heart failure and reduced left ventricular ejection fraction

Abstract

The aim — to determine the prognostic value of weight loss (WL) and body tissue components in patients with chronic heart failure (CHF) and reduced left ventricular (LV) ejection fraction (EF). Materials and methods. We examined 77 patients with stable CHF aged 18 — 75 years, II — IV NYHA class, with LVEF £ 35 %. All patients underwent general clinical examinations and assessment of the component composition of the body using dual energy X‑ray densitometry. Results and discussion. The patients were divided into two groups depending on the WL for the last 6 months: the first group — patients with WL ³ 6 %, and the second group — WL < 6 %. The first group had a significantly worse prognosis for the survival and onset of a combined critical event (death or hospitalization) compared to the second group. The prognostic value has been established regarding the survival of the muscle mass index of the extremities and the ratio of non‑adipose soft tissue and growth. The adipose tissue index and the percentage of body fat in males had a statistically significant effect on the risk of death. No prognostic significance of bone mineral mass relative to survival and the onset of a combined critical event was revealed. Conclusions. In patients with CHF and reduced LVEF, a loss of body mass i 6 % over the past 6 months, a low index of extremity muscle mass and a lower ratio of non‑adipose soft tissue and growth are associated with poorer survival. The adipose tissue index and percentage of adipose tissue correlate to survival in male patients. The indicators of the bone component did not demonstrate informativeness regarding the long‑term survival and the onset of the combined critical event in patients with CHF and reduced LVEF.
