Rapid Palatal Expansion with dental and skeletal anchorage – a literature review

Abstract
Treatment for maxillary narrowing involves palatal suture expansion. Appliances with dental anchorage used to be used for this purpose; however, recently (since 1999) appliances with skeletal anchorage, namely transpalatal distractors TPD (Transpalatal Distraction Osteogenesis) have been used. [b]Aim[/b]. The paper aimed to review current literature regarding appliances used for rapid maxillary expansion with dental and skeletal anchorage. [b]Material and methods.[/b] Based on the literature from the period 1994– 2014 found as a result of a search in an electronic MEDLINE database with medical information, indications and contraindications, as well as advantages and disadvantages of both techniques have been presented. [b]Results and conclusions.[/b] It was difficult to achieve maxillary expansion with appliances with dental anchorage in patients after a growth spurt or without a sufficient dental support and with severe maxillary narrowing – in such cases TPD turned out to be an effective method to provide scheduled expansion rapidly and effectively. In the case of RPE appliances with skeletal anchorage force is directed to the bone, and appliances with dental anchorage transfer force on the bone and teeth as well. However, both methods are associated with complications. In the case of appliances with dental anchorage they include extrusion of lateral teeth in the maxilla and their inclination, and gingival recessions. When TPD is used bleeding and swelling might develop during a procedure of distractor placement, and its loosening or translocation following a procedure.
