Rational therapy of different clinical forms of onychomycoses

Abstract

The review considers the problem of onychomycosis — a fungal infection of the fingernails and toenails. Onychomycosis affects about 10—12 % of the total population. There are five forms of onychomycosis depending on the morphological patterns and degree of invasion in the nails. In choosing a rational therapy for various forms of onychomycosis, an accurate clinical diagnosis, based on clinical signs and laboratory diagnostics, is crucial. In the treatment of onychomycosis, systemic monotherapy, topical monotherapy with or without removal of the nail plate and combined therapy are used. Among drugs the most common for systemic therapy are antifungal preparations of the group of azoles and allylamines. Azols have a fungistatic effect, limit the rate of fungal division by increasing the permeability of the cell membrane. Preparations of allylamines (terbinafine, naphthymine) have not only a fungistatic effect caused by inhibition of squalene epoxidase enzyme, but also a fungicidal action, directly kill the fungi by accumulating squalene in the cells. In many clinical studies, it has been proven that naphthiphine (Exoderil® solution) from the group of allylamines is highly effective and is active against all major pathogens of onychomycoses. The aqueous solution Ekzoderil® is able to penetrate quickly and freely into the channels and cracks of the nail plate, reaching high concentrations directly in the fungi accumulation zone. Exoderil® varnish (amorolfin 5 %) penetrates through the nail plate and has a wide range of activity against dermatophytes, yeasts, mold fungi. Exoderil® varnish can be used to treat onychomycoses as a monotherapy, as part of combined therapy and for the prevention of relapses.
