Register of percutaneous coronary interventions. Are changes for 2015 — 2018 a casual splash of activities or system transformations?

Abstract

The Register of Percutaneous Coronary Interventions (PCI) was created in 2010 by the all‑Ukrainian non‑governmental organization «Association of Interventional Cardiologists of Ukraine». As of 03.08.2019, 19 h 48 min, 127,498 patient protocols, who underwent coronary angiography, PCI or both procedures simultaneously, were completely filled in the Register. Comparative analysis of work of clinics revealed that during the three‑year period of development of the cardiology intervention sector of Ukraine, the number of coronary angiography in patients with various forms of coronary heart disease increased by 70 % and amounted to 40,964 in 2018, which was 96.5 % more than in 2015 (9,761). Analysis of the data indicates that in 2018 the total number of primary PCIs performed in patients with symptoms of acute coronary syndrome with ST segment elevation (STEMI) was 10,774, which was 103.3 % more than in 2015 (5,300). This has led to a nearly twofold increase in the number of primary PCIs per 1 million of population (286 in 2018 and 142 in 2015). The proportion of patients with STEMI in catheterization laboratories was 56 % of the total number of patients with stents in Ukraine. Compared to the member countries of the European Society of Cardiologists, the deficit of hospitalizations of patients with STEMI symptoms decreased and amounted to 14.6 % in 2018 (total number of hospitalizations with STEMI was 26,059); in 2015 it was 20.6 % (23,954). During the observation period, hospital mortality rates from acute myocardial infarction in almost all regions of Ukraine have changed. The average mortality rate for acute myocardial infarction in 2018 was 12.4 %, and in 2015 — 13.9 % (that is, in the hospital 801 more patients survived in 2018, than in 2015). Thus, the changes over the last 3 years are not an accidental burst of activity, but systemic transformations made possible by the collaboration of hundreds of doctors and administrators.
