Revenge of Henoch and Schönlein: a case of diffuse peritonitis in an adult with IgA vasculitis.

Abstract
IgA vasculitis, formerly known as Henoch-Schönlein purpura, is an IgA-mediated autoimmune vasculitis, usually of children. Adult onset of IgA vasculitis is rare. The article presents a case of a 34-year-old male patient with symptoms of purpuric rash, high fever, renal involvement, myalgia and arthralgia later developing symptoms and signs of diffused peritonitis. The patient was operated on laparoscopically. The procedure revealed presence of serous fluid in the abdominal cavity, oedema and dilation of the small intestine with purpura of its walls. The laparoscopic lavage of the peritoneal cavity with saline and drainage were performed. Further treatment with antibiotics, steroids, proton pump inhibitor and analgesics allowed for uneventful recovery.
