Right Ventricular Free Wall Perforation without Tamponade: A Rare Complication of Permanent Pacemaker Lead Placement

Abstract

Ventricular free wall rupture after implantation of a permanent pacemaker (PPM) is a rare condition that can lead to cardiac tamponade. The rupture usually occurs during the first lead placement, so it is worthy of notice. If cardiac tamponade is seen right after the placement, prompt drainage is a lifesaving intervention. Fatal results may be seen due to ventricular perforation. Corticosteroid usage, long stretched pacemaker lead, bipolar electrodes or high body mass index are the most common causes of the perforation. When perforation is determined immediately after the lead placement, new leads should be placed under echocardiography rather than withdrawing. Here we present a patient with free wall rupture caused by the former internal PPM lead that we took out during external pacemaker lead placement during surgery.
