Role of endoscopy and ligation in prophylaxis of complications of endoscopic removal of large intestine polyps

Abstract
The aim of this study was to compare the efficacy of prophylactic clip and endoloop application in the prevention of postpolypectomy bleeding in large pedunculated polyps. A total of 167 patients, who had 217 pedunculated colorectal polyps, with heads ≥10mm and stalks ≥5mm in diameter, were included in the study. Polyps were randomized to receive either standard polypectomy, clips or endoloops. Both devices were applied to the base of the stalk before conventional snare polypectomy. A total of 217 polyps were included in the study (127 in the group with conventional snare polypectomy, 41 in the endoloop group and 49 in the clip group). Bleeding occurred after fourteen polypectomies in the first group (11 %), after one in the endoloop group (2,4 %) and after three in the clip group (6,1 %). Our results suggest that the application of an endoloop is an effective method in the prevention of postpolypectomy bleeding in large pedunculated colonic polyps. In case when it is impossible to apply endoloop, prophylactic clipping can be an alternative method.
