Status of protein metabolism in patients with the first diagnosed pulmonary tuberculosis depending of clinical form

Abstract

Оbjective — to study the state of protein metabolism (PM), namely the content of substituted amino acids (SAA) and essential amino acids (EAA) in blood serum in patients with FDTB of the lungs, depen­ding on the clinical form of TB. Materials and methods. The survey involved 112 patients with FDTB (main group). The patients were divided into 3 subgroups (1st — 21 patients with focal TB, 2nd — 67 patients with infiltrative TB, 3rd — 24 patients with disseminated TB, 30 healthy people of comparable sex and age were the control group. Results and discussion. The smallest changes were detected in focal TB due to the decrease of GCSAA and GCEAA (decrease in the content of certain SAA (glutamic acid, glutamine and alanine) and EAA (lysine, histidine and methionine)). In infiltrative TB, a significant decrease in GCAAs, GCEAA (lysine, histidine, methionine, arginine, phenylalanine) and GCSAA (glutamic acid, glutamine). The largest changes were found in patients with disseminated TB: a significant decrease in GCAA, GCEAA, GCSAA. Conclusions. There is a clear link between the changes in protein metabolism and the prevalence of the tuberculous process in the lungs. The revealed changes are common for all forms of tuberculosis, namely: a decrease in the total amino acid content of the interchangeable (glutamic acid, glutamine, glycine, alanine) and essential (lysine, histidine, methionine) and inherent exclusively for certain forms of pulmonary tuberculosis. For disseminated tuberculosis, the greatest decrease in the total content of amino acids, in particular, interchangeable (glutamic acid, glutamine, glycine, tyrosine, serine) and indispensable (lysine, histidine, methionine, leucine, threonine, arginine) is characteristic. And for patients with infiltrative TB there is a characteristic decrease in symptoms of GCSAA, GCEAA, GCAAs. In patients with focal positions TB and content of most AC remained unchanged or was unreliable reduced. The data reflect the objective state of metabolic balance and PM and is the basis for selecting the method of correcting these violations, in order to improve the treatment of TB.
