Study of AES (Acute Encephalitis Syndrome) with respect to clinical profile and outcome in Patna medical college and hospital

Abstract
Background: Acute Encephalitis Syndrome (AES) is defined as acute onset of fever and altered mental status with and without seizure in a person of any age and any time of the year. There is seasonal and geographical variation in the causative organism. There is high incidence Of AES in Bihar. Objective of this study was to study clinical profile and outcome of AES admitted in the department of Paediatrics PMCH Patna. Method: This study was prospective study done in the Department of Paediatrics in in Patna Medical College and Hospital, Bihar, India from the duration of April 2016 to April 2017. The children who were admitted in the Paediatric ward over a period of one year with clinical features of fever, seizures, altered sensorium were included in the study. The Cases which turned out to be positive for Bacterial / TB meningoencephalitis, febrile seizures were excluded from study. The clinical feature, demography, immunisation and outcome were recorded. The CSF was analysed and IgM Elisa were done for JE, HSV, dengue and CMV. Results: Most common age group with AES was 3 to 7 year. Male to female ratio was 1.6:1. Most common Clinical manifestation was fever with altered Sensorium followed by seizure. Most common outcome was full recovery (57.29%), followed by neurological sequelae (23.95%) and death (18.75%) cases. Etiological diagnosis was JE positive in 9.61%, Enterovirus in7. 6%, Herpes Simplex in 5.76%, and scrub typhus in 3.8% cases. No Etiological diagnosis was established in rest patients. Conclusion: Prompt management of AES results in good outcome. Further studies to establish the etiology of AES were needed for prevention and decrease the burden of AES.
