Subarachnoid Hyperbaric Bupivacaine and Isobaric Levobupivacaine: A Prospective Randomized Double Blind Comparative Study
Journal Title: Indian Journal of Anesthesia and Analgesia - Year 2018, Vol 5, Issue 10
Abstract
Aim: The aim of our study was to compare the efficacy, block parameters and safety profile of intrathecal hyperbaric bupivacaine and isobaric levobupivacaine for urological surgery. Methods: Urological patients who were scheduled for elective surgery under spinal anesthesia were enrolled in two groups. Group A received intrathecal 2.5 ml of (0.5%) hyperbaric bupivacaine, while Group B received intrathecal 2.5 ml of (0.5%) isobaric levobupivacaine. Sensory & motor block parameters, hemodynamic parameters and adverse effects in patients of both the groups were recorded. Results: The onset of sensory block, motor block and duration of motor block was comparable in both groups. Maximum height of sensory block was significantly higher in hyperbaric bupivacaine group and duration of sensory block was significantly higher in group B. Hemodynamic stability was better with levobupivacaine compared to hyperbaric bupivacaine. Hypotension and bradycardia were more common with hyperbaric bupivacaine group. In addition, nausea was noticed more frequently with hyperbaric bupivacaine. Other sideeffects such as headache, backache, itching, vomiting, and shivering were almost similar in both the groups. Conclusion: We conclude that isobaric levobupivacaine is superior to hyperbaric bupivacaine in terms of longer sensory blockade and shorter motor blockade and also that intrathecal use of isobaric levobupivacaine is hemodynamically more favorable than that of hyperbaric bupivacaine.

A Comparative Study of Efficacy of Intrathecal Fentanyl and Nalbuphine as an Adjuvant to Bupivacaine 0.5% Heavy for Lower Limb and Lower Abdominal Surgeries
Introduction: Opioids have been extensively used as an adjuvant because of excellent results in neuraxial blocks. Fentanyl is widely used but is found to have higher incidence of pruritus for which Nalbuphine was conside...
A Comparative Study of Effects of General Anaesthesia and Subarachnoid Block For LSCS in Terms of Maternal and Foetal Outcome in Preeclmaptic Patient
Background: Preeclampsia is hypertensive and multisystem disorder of pregnancy. It is a leading cause of maternal and foetal morbidity and mortality. There is increased rate of lower segment caesarean section in preeclam...
Comparison of Recovery Profile of Sevoflurane and Desflurane in Patients Undergoing Elective Neurosurgical Procedures
Aim: The primary objective of the study was to compare the early post-operative recovery profile of Sevoflurane with Desflurane in adult patients undergoing elective neurosurgical procedures, with respect to emergence ti...
Evaluation of Pre-emptive Intramuscular Glycopyrrolate in Prevention of Spinal Anesthesia Induced Hypotension in Elective Cesarean Sections
Context: Spinal anesthesia induced hypotension in parturients undergoing Cesarean section can be detrimental to both foetus and mother. Prophylactic administration of vasopressors helps in preventing hypotension thereby...
Comparative Evaluation of Butorphanol Versus Nalbuphine for Postoperative Epidural Analgesia in Lower Limb Orthopaedic Surgeries
Background: Epidural opioids acting through the spinal cord receptors improve the quality and duration of analgesia along with dose-sparing effect with the local anesthetics. The present study compared the efficacy and s...
