Surgical treatment of uterine fibroids

Abstract
Uterine fibroids are the most common benign tumors of the genital tract. Pathophysiology of myomas arise is not clear. Although hormons and a number of local growth factors may stimulate myoma development. The genetic basis for fibroid growth may relate primarily to these factors and their receptors. Most women with uterine fibroids are asymptomatic. Nevertheless the management depends on the symptoms, localization, size of the fibroids, and the patients desire to maintain their fertility. Laparoscopic or histeroscopic myomectomy is the best treatment for symptomatic patients with uterine fibroids. Most laparoscopic surgeons have their own criteria for this treatment. Compared to laparotomy, laparoscopic myomectomy has the advantages of small incision, short hospital stay, smaller postoperative pain, fast recovery. Laparoscopic myomectomy is possible if the techniques of laparoscopic suturing skills are managed only. The laparoscopic myomectomy is the best option for future fertility also.Patients with submucous fibroids should be treated via hysteroscopy. The data are not sufficient to determine whether vaginal delivery should attempted, or whether cesarean section should be advised in the group of patients when a myoma distorts the endometrial cavity.
