THE EFFECT OF DURATION AND DIFFICULTY OF WEANING FROM MECHANICAL VENTILATION ON MORTALITY RATE IN LONG-TERM VENTILATED PATIENTS WITH SEVERE TRAUMA

Abstract

Weaning from the respirator is a very important step in the complex intensive care. On the one hand extending the duration of mechanical ventilation leads to a significant increase in risk for the patient, on the other – early weaning may lead to decompensation of respiratory function, premature extubation that requires reintubation. The purpose of our study was to estimate the impact of the duration of weaning from the respirator on the mortality in patients who underwent prolonged mechanical ventilation using the method of adaptive supportive ventilation (ASV). Our prospective study included 25 patients diagnosed with severe trauma, in whom prolonged mechanical ventilation (more than 120 hours) was conducted. The mode of adaptive supportive ventilation was used for weaning. The mode of ASV appeared to be an effective method for weaning in patients after long-term mechanical ventilation. Frequency of successful weaning was 56%, necessity of reinitiating mechanical ventilation occurred in 32% of patients. Mortality rate was higher in patients with difficult and long-term weaning. Further studies are needed to improve the treatment outcomes in this group of patients.
