The effectiveness of immunoglobulin-G in patients with DRTB/HIV in a state of expressive immunosuppression according to hematological studies

Abstract

Objective — to study hematological disorders in patients with co­infection of DR TB/HIV in a state of significant immunosuppression, and to evaluate the effectiveness of immunoglobulin therapy by IgG. Materials and methods. 52 patients with DRTB/HIV aged from 20 to 55 years with CD4 lymphocyte counts up to 50 cells/µl were examined. Patients were divided into two groups: 1st (control) — 26 patients received standard treatment with second­line antituberculosis drugs with antiretroviral therapy; 2nd (main) — 26 patients received additional intravenous IgG in complex therapy. Results and discussion. Before the beginning of antituberculous therapy, leukocytosis was observed 3.2 times more in patients of both groups than leukopenia (50 and 15.4 %, respectively); lymphopenia was 7.3 times higher than leukocytosis in the first group and 9.3 times in the second group. Monocytopenia was noted in (76.9 ± 1.69) %, monocytosis in (15.4 ± 1.44) % of patients of the first group and monocytopenia (73.1 ± 1.77) and monocytosis (15.4 ± 1, 44) % in the second group. That is, almost five times more often monocytopenia was registered. Eosinophilia was determined in both groups with a frequency (15.4 ± 1.44) %. The level of neutrophils increased in (69.2 ± 1.85) and (73.1 ± 1.77) % of patients in the first and second groups, respectively. Anemia of mild and moderate degree was recorded in the same amount — in (38.5 ± 1.95) % of patients of the first and second groups, and severe anemia in (11.5 ± 1.28) % in the first group and (15.4 ± 1.44) % — in the second. Thrombocytopenia was diagnosed in (34.6 ± 1.91) and (38.5 ± 1.95) % of patients in the 1st and 2nd groups, respectively. Conclusions. In patients of group 1, after 2 weeks of treatment, leukocytosis, neutrophilia, and lymphopenia were observed in patients of the 2 group, because of adherence to the course of intravenous IgG treatment, a decrease in the level of leukocytes and neutrophils was observed. Also, there was a decrease in the manifestations of anemia, due to a decrease in the toxic load on the organs of hematopoiesis.
