THE INFLUENCE OF EARLY CRANIECTOMY AND MICROSURGICAL TREATMENT OF RUPTURED MCA ANEURYSM ON NEUROLOGICAL RECOVERY ACCOMPAINED WITH EXTRACRANIAL COMPLICATION
Journal Title: INTERNATIONAL JOURNAL OF PURE MEDICAL RESEARCH - Year 2018, Vol 3, Issue 10
Abstract
AIM: To emphasize the importance of early recognition, diagnostic processing and emergent surgical treatment of spontaneous rupture of intracranial aneurysms (aSAH) METHODS: A 41-year-old female presented with sudden impaired state of consciousness up to coma. "A golden hour" native computed tomography (CT) scan showed signs of voluminous subarachnoidal hemorrhage (SAH) into basal cisterns lled with hemorrhagic content (Fisher grade III) and patient was sent to referral neurovascular centre due suspicious right MCA aneurysmatic rupture. RESULTS: Repeted native CT scan showed signs of subarachnoidal hemorrhage (SAH) into basal cisterns, intracerebral hematoma into right temporal lobe accompanied with a cloak of right subdural haemathoma (Fisher grade IV). CT angiography scans (CTA) conrmed aneurysmatic rupture at bifurcation of right MCA. The patient underwent emergent decompressive craniectomy and clipping of ruptured aneurysm within 6 hours of symptoms onset. CONCLUSION: Aneurysmal subarachnoid haemorrhage (aSAH) is a devastating condition that we should to think about in patients presenting with sudden impaired state of consciousness up to coma like it was in our case. Computed tomography (CT) and CT angiography (CTA) present a gold standard and should be routinely performed in order to exlude or conrm the presence of ruptured intracranial aneurysm. Prompt surgical decompression and occlusion of ruptured aneurysm is an absolute surgical indication.

EXTRACT OF THE MEDICINAL PLANT PAO PEREIRA INHIBITS PANCREATIC CANCER STEM CELL IN VITRO AND IN VIVO
Pancreatic cancers are enriched with cancer stem cells (CSCs), which are resistant to chemotherapies, and are responsible for tumor metastasis and recurrence. Here we investigated an extract from the medicinal plant Pao...
EFFECT OF EXERCISE ON HEART RATE RECOVERY INDEX IN NORMOTENSIVE OFFSPRING WITH FAMILY HISTORY OF HYPERTENSION
Objectives: The relationship between autonomic system and cardiovascular mortality is signicant and this study can be used to study the hereditary risk that an individual carries to develop any autonomic dysfunction and...
A RARE CASE OF MUCOCELE CONFINED TO THE RIGHT PTERYGOID PROCESS.
Mucoceles are benign, slow-growing, locally expansive masses within cavities and lled with mocous. They typically arise in the paranasal sinuses and the frontal and ethmoidal sinuses are the most frequent location. We p...
ELECTROCUTION OF MOSQUITOES IN A PIGGERY BY A NOVEL ELECTROSTATIC WINDOW SCREEN TO MINIMIZE MOSQUITO TRANSMISSION OF JAPANESE ENCEPHALITIS VIRUS
A novel electrostatic window screen was devised for installation in the windows of a piggery to prevent house mosquitoes (Culex pipiens) from transmitting Japanese encephalitis virus (JEV) to the surrounding human popula...
WIDE COMPLEX TACHYCARDIA IN ADULT PATIENT WITH EBSTEIN'S ANOMALY: WHAT IS THE MECHANISM?
We present the case of a 60-year-old female with a history of Ebstein's anomaly diagnosed at childhood admitted to our hospital for wide complex tachycardia. Programmed ventricular stimulation was negative. Electrophysio...
