The investigation of herbal medicine Holelesan® in patients with functional pathology of biliary system and intestine

Abstract

Objective — to investigate efficacy of the herbal drug Holelesan® in the treatment of patients with functional diseases of the biliary tract (FDBT) in combination with irritable bowel syndrome with constipation (IBS-C). Materials and methods. The examinations involved 60 patients with IBS-C in combination with FDBT (gallbladder dysfunction) aged 18 to 65 years (mean age 39.2 ± 3.1 years). Among them, there were 41 women (68.3 %) and 19 men (31.7 %). The diagnosis of IBS-C and FDBT was established on the basis of the Rome IV diagnostic criteria. Patients were allocated into two groups, 30 patients in each: subjects of the first group received Holelesan® (Arterium production) 2 capsules 3 times a day 30 minutes before meals, patients in the second group took artichoke preparation in a dose of 400 mg 3 times a day. The duration of treatment was 20 days. A number of investigations and assessments were performed at the baseline and on the 21st day of treatment: the frequency of bowel movements, the nature of the stool according to the Bristol scale (BS), the severity of abdominal pain on a visual analogue scale (VAS), the contractility of the gall bladder was determined sonographically during the food load test. The syndrome of intestinal bacterial overgrowth (SIBO) was assessed with lactulose H2-respiratory test. Results. After 3 weeks of treatment, patients in the first group demonstrated an improvement in their condition, manifested in a decrease in the intensity of pain by 51 %, up to 3.5 ± 0.8 cm on VAS (p = 0.01), absence of pain was reported by 23.3 % of patients. The frequency of defecation doubled to 2.4 ± 0.6 times/week (p = 0.08), and the shape of the stool in BS reached 3.3 ± 0.6 (p = 0.045). The stool of 3 — 4 type was defined in 56.7 % of patients. Moreover, an improvement in the contractile gallbladder function was established, while the gallbladder ejection fraction reached 45.3 ± 6.1 % (p = 0.05), and biliary sludge was significantly reduced and was observed in 23.3 %, which was initially present in 40 % of patients. The SIBO rate decreased almost twice after the treatment (in 43.3 % of patients in this group p > 0.05). The improvement was also registered in patients of the second group. The VAS pain intensity decreased by one third and amounted to 4.6 ± 0.8 cm (p = 0.06) (absence of pain was reported by 10 % of patients). The frequency of bowel movements in this group increased by 50 % and amounted 2.1 ± 0.4 times/week, (p = 0.17), the shape of the stool in BS corresponded to 2.2 ± 0.5 (p = 0.37), normal stool was noted by the end of the study in 26.7 % of patients. The gallbladder ejection fraction in patients of this group was 43.8 ± 6.0 % (p = 0.047). After treatment, the SIBO was defined in 63.3 % of patients (p > 0.05). Conclusions. The natural herbal multicomponent medication Holelesan® has a significant cholekinetic, laxative and prebiotic effects and can be recommended for the treatment of the functional pathology of the biliary tract and irritable bowel syndrome with constipation.
