The most common errors in the diagnosis and management of haemorrhoids

Abstract

Haemorrhoids are the most common anorectal condition. They affect approximately 4% of the adult population, and the symptoms associated with this condition are reported by approximately 50% of patients above the age of 50. The disease is so common that most patients and even some physicians relate all anorectal symptoms to haemorrhoids. This can lead to diagnostic pitfalls. The most common diagnostic errors include: failure to examine the patient and conduct a colonoscopy prior to haemorrhoid treatment, mistaking haemorrhoids for anal vein thrombosis and misdiagnosing prolapse of the rectal mucosa or other perianal conditions (neoplasia, polyps, warts). When selecting the method of treating haemorrhoids, several factors must be included, such as age and general health of the patient, type and character of symptoms, stage of the disease, the patient’s profession and their psychological attitude to the suggested method of treatment. Ignoring these factors can lead to a recurrence of haemorrhoids and complications. The most common errors made during the management of this disease are: too hasty qualification for a surgery with no attempt to implement conservative treatment, failure to adjust the treatment method to the stage of the disease as well as uncritical application of novel surgical techniques without the consideration of indications and possible complications.
