The Prevalence of Keratomycosis, Dermatophytosis and Onychomycosis in a Tertiary care Hospital

Abstract
Background: In recent years, fungi have been emerging more common infection in immunocompromised patients of tertiary care centers. Objective: The retrospective analysis for a period three years was undertaken to know the prevalence of keratomycosis, dermatophytosis and onychomycosis in a tertiary care Hospital. Materials and Methods: The samples were collected from the clinically suspected cases. The direct microscopy with potassium hydroxide (KOH) was done to visualize the presence of fungal elements and Gram staining was done for any suspected yeast infection. Fungal culture of all samples were inoculated on two different culture media; one sabourauds dextrose agar (SDA) and the other SDA with chloramphenicol and cycloheximide. Results: A total of 633 clinical samples suspected fungal etiology were included for a period of three years. Out of which corneal scraping was most common sample 478(75.51%), followed by skin scraping 105(16.59%) and nail clipping 50 (7.9%). Male to Female ratio of patients is 1.55 and majority of the patients belong to age group 21-50 years. Fusarium species was the predominant fungal isolate 57(23.65%), followed by Aspergillus flavus 29(12.05%), Aspergillus niger 28(11.62%), Aspergillus species 21(8.71%), Aspergillus fumigatus 20(8.3%) and Trichophyton species 20(8.3%). Conclusion: The wet mount preparation of KOH can be used as a screening test for presumptive diagnosis of fungal infections. Laboratory confirmation should be undertaken and fungal infection must be ruled out before prescribing corticosteroids and antibacterial agents.
