The use of the appliance with a Hyrax screw in the treatment of patients with maxillary constriction

Abstract
[b]Introduction.[/b] Expanding the palatal suture is a method applied to treat significant maxillary constriction. The goal of the treatment is to expand the maxillary base through expanding the palatal process of the maxillary bones and horizontal plates of the palatal bones, and then to maintain the width until a new bone will fill the gap. There are two methods to expand the palatal suture: RPE (rapid palatal expansion) and SPE (slow palatal expansion). Nowadays, appliances with a Hyrax screw are most frequently used for a rapid maxillary expansion. Appliances with a Hyrax screw can be equipped with a range of additional elements useful in treating the variety of malocclusions accompanying maxillary constriction. [b]Material and methods[/b]. Thirty-one patients (19 girls and 12 boys) treated in the Specjalistyczna Poradnia Ortodoncji Wojewódzkiej Przychodni Stomatologicznej im. dr n. med. Zbigniewa Żaka in Cracow between 2010-2011 were included in the study. Patients were treated because of narrow maxilla with unilateral or bilateral cross bite. The screw activation was continued to achieve a contact of palatal cusps of upper molars with labial side of buccal cusps of lower molars. Measurements on the plaster models were performed to evaluate the changes which were achieved during the treatment. [b]Discussion[/b]. The expansion of the palatal suture is an effective method of maxillary constriction treatment in patients with class II and III, with unilateral and bilateral cross bite. Because of using acrylic bite-blocks it is possible to use the appliance in high-angle patients with vertical growth type, long face syndrome, small overbite, tongue pressing to avoid lateral tooth extrusion. Analysis of results of this study shows that in all our patients the palatal suture expansion was achieved. (Orthod. Forum 2012; 8: 115-123). Received: 8.06.2012 Accepted: 18.09.2012
