Three-year antibiotic stewardship in a Greek maternity hospital

Abstract
OBJECTIVE Antibiotic use monitoring is essential for implementing a guided antibiotic policy. In this maternity hospital an antimicrobial stewardship programme was initiated in order to upgrade the high standard of care, reduce costs and establish a hospital pharmacy database. METHOD The data collection was based on the electronic data of the hospital pharmacy that documents all the prescribed antibiotics. The pharmacy consumption data were categorized according the ATC code and were processed by the WHO Defined Daily Dose methodology (DDDs/1,000 occupied bed-days) for the 6 Obstetrics-Gynecology Clinics from January 2005 to December 2007. RESULTS The total hospital antibiotic use showed no fluctuations of statistically significant difference throughout the 3-year period (p=0.892). The largest segment of antibiotic consumption was recorded for the penicillins and cephalosporins, and the lowest for the carbapenemes, sulfonamides, lincozamides, quinolones and glycopeptides. The total use of quinolones was very low (60.7 DDDs), and was constant throughout the 3 years. The use of carbapenemes and glycopeptides was recorded only in the year 2007. CONCLUSIONS The results of the monitoring were exploited for the implementation of appropriate strategic measures for the control of antibiotic usage. Continuing measurement and guidance are essential for antibiotic prescription control.
