BACKGROUND One of the world’s largest community-based outreach scheme, Integrated Child Development Service (ICDS) is working to improve the undernutrition in the child with its vast infrastructure amounting routine analysis to guide the ongoing efforts to further improve its effectiveness. The objective of this study is to assess the difference in reported undernutrition among children in a health block of rural and urban area of Northern State of India. MATERIALS AND METHODS Routine report analysis of supplementary nutrition of children under five years of age for year 2015 of study areas; the urban block, Nagrota Bagwan, Kangra and rural block Haroli, Una, Himachal Pradesh. RESULTS In urban study area, across all the age groups it was observed that moderate undernutrition (26.1%) was reported relatively more than the rural block (11.4%) with a declining trend over the twelve months in rural area. No such trend was observed in urban block. Severe malnutrition was also reported to be higher in rural block and in female children. CONCLUSION Nil reporting of severe undernutrition in rural block with the reported low prevalence of moderate undernutrition in urban block as compared to rural is indicative of self-reported success of the programme in one area. Limitation of information from any other source for undernutrition in the studied block and a comparison of state level information from nationwide survey indicated a reporting bias (under-reporting) in the area and require adjudication by an independent survey.
