Uygulamada Selektif Lazer Trabeküloplastiye Karşı Timolol İçeren Fiks Kombinasyonlar: Primer Açık Açılı Glokomda Replasman Çalışması

Abstract
Amaç: Bu çalışmada primer açık açılı glokomda 360˚ ve 180˚ selektif lazer trabeküloplasti (SLT) tedavisi seçeneklerinin 6 aylık süreçte etkinlik açısından timolol içeren fiks kombinasyonlarla (FK) değiştirilebilirliğinin değerlendirilmesi amaçlandı. Gereç ve Yöntem: Prospektif, karşılaştırmalı, girişimsel olgu serisi olarak dizayn edilen çalışmaya 40 hasta dahil edildi. Bu hastalara ait 18 göz SLT 180˚ tedavi alt grubuna, 22 göz de SLT 360˚ tedavi alt grubuna randomize edilirken, aynı ilaçları kullanan ve göz içi basınçları (GİB) özdeş olan diğer gözler de kontrol grubuna dahil edildi. SLT uygulanan gözlerde tedavinin hemen ardından timolol içeren FK’ler kesilirken, kontrol gözlerinde ise aynı şekilde devam edildi. Lazer sonrası 1. saatte ve 1. günde GİB yükselmesi değerlendirildi. GİB’in ve olası diğer komplikasyonların değerlendirildiği takip muayeneleri 1. hafta,1. ay, 3. ay ve 6. ayda gerçekleştirildi. Bulgular: Lazer sonrası 1. saat ve 1. gün hariç (sırasıyla p<0,001 ve p=0,010) 6 ay boyunca ortalama GİB değerleri açısından gruplar arasında anlamlı fark izlenmedi. Çoklu karşılaştırma analizinde GİB, gerek SLT 180˚ ve gerekse SLT 360˚ alt gruplarında, kontrol gözlerinden istatistiksel olarak 1. saatte yüksek iken (p=0,007, p<0,001); yalnızca 1. günde SLT 360˚ grubunda kontrol gözlerinden istatistiksel olarak düşüktü (p=0,013). Sonuç: SLT, GİB düşürme etkinliği açısından timolol içeren FK’lerle eşit etki gücüne sahip gözükmektedir. Ne var ki, SLT 360˚ uygulaması ilave etki sağlamayabilir.
