VARIANT ANATOMY OF THE PULMONARY VENOUS ORIFICES AND THEIR CLINICAL SIGNIFICANCE

Abstract
Interest in the study of pulmonary veins and its ostia has considerably increased in recent times after the realization of the fact that atrial fibrillation may originate from the triggers in the pulmonary veins as they receive the extension of myocardial sleeves from left atrium. Researchers have found that the classic pattern of two ostia in the either side of the left atrium occurs in 70% of general population. The variations pulmonary venous drainage patterns are mostly congenital in origin and related to the complex development of venous system. AIM: The pulmonary venous ostia were studied with respect to their number and orientation. METHODS AND MATERIAL: The present study was carried out in the Department of Anatomy, Jawaharlal Nehru Medical College Sawangi, Wardha, Maharashtra and Subharti Medical College Meerut Uttar Pradesh on 51 formalin fixed heart specimens. They were obtained from cadavers belonging to the age group of 50-75 years. RESULTS: Numerical variability was found in form of supernumerary or lesser number of ostium. Twenty five (49.01%) specimens had four pulmonary venous ostia, nine (17.65%) specimens had five ostia, four (7.84%) specimens had six, one (1.96%) specimen had seven, eleven (21.57%) specimen had three and one (1.96%) specimen had only two ostia.The ostia of the superior pulmonary veins of both sides were found to be directed upwards and laterally and the inferior pulmonary veins of both sides backwards and laterally. The middle pulmonary vein whenever present was directed laterally irrespective of the side to which it belonged. The ostia of the additional pulmonary veins were inconsistent as far as location and orientation was concerned. CONCLUSION: Considerable variations were found in this study in terms of additional and less number of pulmonary venous ostia. Variations were found more on the right side. Knowledge of the usual and variant anatomy of the pulmonary veins and ostia is very valuable for the success of radiofrequency catheter ablation and safe execution of other cardiothoracic and radiological invasive procedures, undertaken for various therapeutic purposes, in this region.
