A CLINICAL AND ELECTROENCEPHALOGRAPHIC PROFILE OF FIRST UNPROVOKED SEIZURE IN ADOLESCENTS AND ADULTS IN A TERTIARY CARE CENTER

Abstract
Context:A first unprovoked seizure is frightening experience both for the patient and family. Prognosis and treatment of the first unprovoked seizure depends on identification of specific epilepsy syndrome. Aims:To study the clinical and electroencephalographic profile of first unprovoked seizure in adolescents and adults.Settings and Design:It is a cross sectional descriptive and retrospective study of patients with first unprovoked seizure.Methods and Material:30 consecutive patients presenting with first unprovoked seizure at Neurology department, Government Stanley medical college between January 2016 and august 2016.Inclusion criteria:All patients aged 13 years or older, presenting with first unprovoked seizure.Exclusion criteria:Patients with non-epileptic seizure or those with acute symptomatic seizure.Statistical analysis used: SPSS software.Results:The mean age of presentation was 27.5 years with a slight male preponderance (male- 56% female-43%). Based on clinical semiology a diagnosis of primary generalised epilepsy was made in 10%, partial epilepsy in 43% and unclassified seizure in 46.66% of cases. All of those with partial seizure was of extratemporal semiology. EEG was abnormal in 23% and was normal in 76.66% of cases.Conclusions:Most common age group affected was 13- 20 years. Males were slightly more affected than females( 56% vs 43%). Most common seizure subtype was unclassified seizure type. Juvenile myoclonic epilepsy was the most common primary generalised epilepsy syndrome. EEG was abnormal in all patients with primary generalised epilepsy Key Messages: most common age group affected is 13-20 years. While unclassified seizure is the most common type of seizure, JME is the most common primary generalised epilepsy syndrome to be diagnosed. EEG is to be done within 24 hours of a seizure to increase its yield
